We are pleased to highlight the 5th European Symposium on Transition, taking place in Paris, France, on 25-26 September 2025.
Held every two years, the European Symposium on Transition brings together researchers, clinicians, and professionals from across disciplines to exchange insights and strengthen practices in the field of transitional care for young people with chronic conditions or disabilities. Following successful editions in Lausanne and Rotterdam, this next symposium will continue to foster learning and collaboration on a European scale.
Who Should Attend?
This symposium is particularly relevant to:
- Healthcare professionals (e.g. physicians, nurses, psychologists, physiotherapists, social workers) working with adolescents and young adults in somatic care, rehabilitation or mental health
- Researchers seeking to exchange findings and explore emerging themes in transition care
- Patient organisations and youth advocates
- Healthcare administrators and decision-makers with an interest in integrated care pathways
The 2025 edition will focus on current challenges and outstanding questions in the field, offering a valuable opportunity to learn from both practice and research. It will also provide a platform to strengthen networks and promote collaboration across countries and disciplines.
